If you would like to become a Unity Selections customer, please contact us at (303) 800-3600 or via email at info@unity-selections.com. All we need to get the process started is a completed New Account Application, a Credit Application and a copy of your current State of Colorado Liquor license. A completed Credit Application and copy of your Liquor License can be faxed to us at (720) 389-6316.
